Secondary Piggyback IOL Implantation for Correction of Residual Refractive Error Following Cataract Surgery

Purpose
To report the visual and refractive outcomes following secondary piggyback intraocular lens(IOL) implantation to correct residual refractive errors following cataract surgery.
Methods
Retrospective interventional case series. 11 eyes of 11 pseudophakic patients who had the primary IOL in the capsular bag during cataract surgery underwent planned secondary piggyback IOL implantation in the ciliary sulcus to correct residual refractive error. Data including post operative refraction following primary implant surgery, pre operative uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA) and manifest refraction were collected. Following secondary piggyback IOL implantation, data on the design and type of piggyback lens implant, postoperative UDVA and CDVA were collected.
Results
There were no intraoperative or postoperative complications. For all patients in the cohort the UCVA improved from 0.73 (LogMAR) preoperatively to to 0.22 (LogMAR) postoperatively with a mean improvement of 0.51 (LogMAR). Seven patients (64%) achieved a CDVA of 0 (LogMAR) or better postoperatively. The mean spherical equivalent (MSE) improved was from -1.10 diopters (D) (range -7 to +3.25D) preoperatively to -0.27 D (range -1 to +0.375 D) postoperatively. Eight patients (73%) were within +/-1.00 D of emmetropia.
Conclusion
Secondary piggyback intraocular lens is a safe and effective method to correct residual refractive error following cataract surgery.
