Blockage of Glaucoma Filtration Device by Microlens Particles After Cataract Surgery

Purpose
To report a case of blockage of glaucoma filtration device due to micro lens particles following phacoemulsification.

Methods
A female aged 36 years having pigmentory glaucoma who was operated for trabeculectomy earlier, had uncontrolled glaucoma with advance cataractous changes with flat bleb. Hence Phaco with IOL and antiglaucoma surgery with glaucoma filtration device (Ex-PRESS implant) at different sites in same sitting was carried out. IOP remain controlled for only one month. Then it went up due to blockage of tunnel of GFD by micro lens particles which was confirmed by gonioscopy. During next surgery, as the Ex-PRESS implant tunnel found blocked and could not be opened up, another Ex-PRESS implant was inserted with conjunctival autograft.

Results
Eye remained quite and AC remained formed from day one. IOPr remained between 12 to 15 mm of Hg after one week, one month, 3 months and 6 months without any antiglaucoma medications. Bleb has formed very well by one month. Post op. corrected visual acuity was 20/40 after 6 months.

Conclusion
Micro lens particles due to Phacoemulsification can block the GFD. In this case, another implant (GFD) could control the IOPr successfully.
