Postop rise of IOP and TASS are complications of OVDs.  Video shows a technique of phaco without OVDs.  Incision, capsulorhexis, and IOL implantations are done under balanced salt solution provided constantly by the irrigating cannula.  A prospective randomized study compared effect of presence or absence of OVD on postop IOP, inflammation, and endothelial cell loss and showed increased inflammation on first postop day in OVD group.  Intraocular pressure and endothelial cell loss were similar in both.
