Outcomes of Phacoemulsification and Implantation of High Refractive Power IOL in Nanophthalmos

Purpose
To evaluate the refractive outcomes and postoperative results for patients undergoing phacoemulsification and implantation of a high refractive power intraocular lens (Zeiss CT Xtreme D) for cataract surgery or clear lens exchange with or without angle closure.
Methods
We conducted a retrospective chart review to identify patients (n = 20) who had a diagnosis of nanophthalmos and underwent phacoemulsification and placement of a Zeiss CT Xtreme D intraocular lens (IOL) for cataract surgery at 5 Canadian sites. Non-identifying descriptive data and clinical data were extracted from the charts. Outcome measures included patient demographics, corrected distance visual acuity (CDVA), refraction, glaucoma diagnosis, intraocular pressure (IOP), glaucoma medication use, follow-up time, procedures and complications. These parameters were also compared pre- and post-surgery. Additional data included axial length (AL), anterior chamber depth (ACD), corneal keratometry and IOL power used.
Results
Twenty eyes with a mean follow up time of 185 days (range 60-390) were studied. Mean age was 58 years (range 27-79). The mean IOL power (D) implanted was 50.2 ± 3.9. The mean preoperative IOP (mmHg) was 14.5 ± 7.3 on 1.0 ± 1.7 glaucoma medications. The mean postoperative IOP was 11.5 ± 3.5 mmHg on 0.7 ± 1.0 medication. The mean AL (mm) was 17.6 ± 2.4 (range 16.1-22.5). The mean CDVA improved from 0.7 ± 0.4 to 0.5 ± 0.3 logMAR. The mean spherical equivalent (D) improved from +14.7 ± 5.4 preoperatively to +2.9 ± 2.0 postoperatively.
Conclusion
Evaluation of patients undergoing phacoemulsification and implantation of the Zeiss CT Xtreme D intraocular lens for cataract surgery or clear lens exchange with or without angle closure yields good results. Additional results pending. Evaluation of this IOL has not been published to date.
