Purpose
To study the long term refractive and visual outcomes of photorefractive keratectomy (PRK) with intraoperative application of mitomycin C (MMC) for the high myopia.

Methods
This study included 44 eyes who received PRK for high myopia; after photoablation, 0.02% MMC solution was applied in all corneas for 30 seconds. Efficacy, safety, predictability, and stability of PRK MMC were evaluated.

Results
Th study includes 44 eyes of 22 patients. Mean preoperative spherical equivalent (SEQ) was −6.50 ± 1.75 D (diopters)(range from -5,00 to -9,00 D) and reduced to −0.35 ± 0.15 D at the last postoperative examination. Mean follow-up was 64.73 ± 16.22 months(range from 72 to 96). No eyes did lose CDVA. At the last postoperative interval, all corneas were found clear.

Conclusion
PRK and MMC for the high myopia, had good predictability and safety for up to 96-month follow-up without serious complications.
