Post-RK Refractive Surprise Managed by IOL Exchange Aided by ASCRS Calculator

Purpose
To report a case of a patient who had previous history of radial keratotomy (RK) and had cataract surgery elsewhere . The patient had a postoperative refractive surprise of +8.00 diopter and requested for an intraocular lens (IOL)  exchange . The ASCRS calculator was used to determine the IOL power and IOL exchange was performed

Methods
This is a retrospective case study of a middle aged man who reported with refractive surprise of +8.00 diopter after cataract surgery elsewhere.He was implanted +5 diopter IOL and his best corrected visual acuity (BCVA) was 6/18 n6  . The patient was subjected to oculyzer evaluation and the parameters were inserted in the online ASCRS calculator and IOL power was determined . Patient had the IOL exchange based on this power (19 diopter) . After 1 month patient was examined for refraction.

Results
At 1 month postop, refraction showed the value of  +0.50DS  / -2.50 DC @ 20 and add of +3.00 diopter for near. His BCVA was 6/18 n6

Conclusion
IOL calculation in post RK eyes can be challenging issue. The use of online ASCRS calculator can help in determining correct IOL power.
