Combined Posterior Phakic IOL and Small-Incision Lenticule Extraction in Highly Myopic Patient

Purpose
To describe the clinical outcome of a high myopic patient who underwent  a combination of foldable posterior phakic intraocular lens (pIOL) implantation and the small incision lenticule extraction (SMILE) procedure.

Methods
Case report with visual refractive and topographic changes before and after both surgeries.

Results
After the implantation of the  pIOL the manifest refraction in OD was -2.50 - 0.75 x 30 with a CDVA of 20/25. The patient successfully underwent a femtosecond SMILE procedure in the same eye 5 months later. After the SMILE, the manifest refraction in OD was N – 0.25 x 90 with a UDVA of 20/25.

Conclusion
This case demonstrates the possibility of performing

Femtosecond-Only ReLEx Smile,  after pIOL implant with good visual outcomes, without complications related to the suction of the femtosecond procedure.
