Management of Collagen Copolymer Phakic IOL Sizing Discrepancies With UBM

Purpose
To correlate the Sulcus to Sulcus (STS) distance and the predicted vault measured preoperatively on the UBM with the actual post-operative vault following ICL implantation

Methods
UBM was performed preoperatively to measure the sulcus to sulcus distance and the anterior chamber depth. This particular model of UBM has an ICL simulator software which provides the predicted vault of the ICL. 6 weeks post-operatively the achieved vault measured on the Anterior Segment Optical Coherence Tomography (ASOCT) was compared with the UBM Sulcus to Sulcus Distance and the predicted vault

Results
The Mean UBM predicted vault was 0.58 ± 0.138 SD and post-op vault on ASOCT was 0.66 ± 0.214 SD. The Bland Altman plot showed variation in White To White (WTW) and Sulcus To Sulcus in more than 50% cases .The paired T-Test showed significant correlation between the  predicted vault on UBM and  post-op vault on ASOCT (p=0.015)

Conclusion
The UBM is an invaluable tool in determining the appropriate ICL diameter and in the objective assessment of vault in the post-op period, especially in eyes where there is poor correlation between WTW and STS. Predicted vault determination on UBM further simplifies sizing dilemmas
