Purpose
To evaluate safety and efficacy of SMILE over follow-up for high myopia more than -10 D.

Methods
Prospective non comparative case series carried out on 285 eyes of 166 myopic patients of errors more than 10D treated with femtosecond laser SMILE. Uncorrected visual acuity (UCVA), best spectacle corrected visual acuity(BSCVA), manifest refraction and contrast sensitivity were measured in all cases.

Results
Mean preoperative UCVA, BSCVA and spherical equivalents were 0.05, 0.67 and -11.93 respectively. All these parameters showed statistically significant change in the postoperative period.

Conclusion
SMILE is a safe and effective procedure with long-term refractive stability for very high myopia.
