History of eye surgery, cornea or retina disorders, ocular dryness were exclusion criteria. The average lens densitometry (ALD) was measured on SS-OCT scans provided by the IOLMaster 700 using an image processsing software. The ocular scatter index (OSI) measured with a double-pass aberrometer, the “Pentacam Nucleus Staging” (PNS) score and mean PNS obtained from anterior segment Scheimpflug tomography were also measured and compared to the ALD.</p><p><b>Results</b><br>One hundred and ten eyes (51 with cataract and 59 controls) of 60 patients aged 21-85 years (mean ± SD: 47.99 ± 21.84 years) were included. ALD measurements were repeatable (P=0.99, ANOVA). The repeatability limit was 2.50 ± SD pixel unit (pu). The average lens densitometry correlated with the OSI (r² = 0.52, P <0.01), PNS score (r² = 0.75, P <0.01) and mean PNS (r² = 0.41, P <0.01). An average lens densitometry greater than 82.9 pu was the cut-off threshold for cataract with 73.9% of sensitivity and 91.2% of specificity. </p><p><b>Conclusion</b><br>The average lens densitometry (ALD) measured by Swept Source-OCT is a repeatable and reliable objective cataract grading method. It correlates with previously validated objective methods including OSI measurement.
