A 50-year-old white woman developed extensive fibrous ingrowth into the anterior segment involving posterior cornea, iris, and anterior lens capsule following acute angle-closure glaucoma. Video highlights steps of extensive anterior segment reconstruction with penetrating keratoplasty, fibrous ingrowth excision, subtotal iridectomy and cataract surgery combined with custom-made artificial iris implantation.
