Alteration in Spherical Aberration of Cornea After Cataract Surgery

Purpose
To study the alteration in corneal spherical aberration value after cataract surgery and to compare with quality of vision.
Methods
170 eyes of 91 cataract patients were examined for keratometry and corneal spherical aberration by newer generation corneal topographer before and one month after uncomplicated cataract surgery. All cases were operated by a single surgeon. 94 eyes had 2.4 mm main corneal incision (Group A) while another 76 eyes had 2.8 mm main corneal incision (Group B). In all cases single piece aspheric foldable IOL was implanted. All cases were subjected to standard questionnaire and vector vision testing to determine visual quality.
Results
Only 4 eyes, SA remained same, otherwise in 166 eyes, with 2.4mm main corneal incision, SA value was increased by 34.14% and flattening of cornea by 0.25% in Group A1, while SA value was decreased by 11.69% and flattening of cornea by 0.24% in Group A2. With 2.8mm main corneal incision, there was 33.90% increase in SA value and 0.08% corneal flattening in Group B1, while in Group B2, SA value decreased by 20.14% and cornea flattened by 0.49%. Though both major groups with SA value change on either side did not show significant disturbances in visual quality.
Conclusion
In this preliminary study, corneal spherical aberration after cataract surgery showed change on either sides i.e. increase and decrease, but in either group, visual quality remain unaffected, which may change the concept about asphericity of whole eye.
