Interesting Case of Collagen Copolymer IOL Exchange

Purpose
To see the result of ICL exchange in right eye of a patient of moderate myopia.
Methods
A patient was implanted with a spherical ICL for moderate myopia after thoroughly investigated for preoperative measurement including BCVA, pachymetry, ACD, corneal diameter etc. ICL implantation was done successfully without any complication. However, after one month postop, it was observed that ICL was almost touching the crystalline lens. Measurements were reassessed and exchange of ICL was considered. After seven months, ICL was exchanged with one larger size ICL.
Results
Exchange of ICL was done successfully without any trouble. Patient got BCVA of 20/20. However, accommodation was restricted due to previous ICL touch. But, it is improving with time.
Conclusion
ICL was successfully exchanged in a patient with moderate myopia in one eye. Despite of thorough preop investigations, undersizing of ICL occurred. One has to be very careful in preop workup.
