A 12-year-old boy presented with blurred vision and red eye for several weeks following swimming in the river.  Slitlamp biomicroscopy showed conjunctival redness adn a live floating microorganism. Other exams were normal.  Under general anesthesia, intracameral acethylcholine (Miochol) was injected to induce miosis to prevent larval migration into the posterior chamber, which led to larval immobilization and death. Larva were then removed from the eye.
