A 20-year-old woman had congenital subluxated cataract surgery. A dense persistent pupillary membrane prevented full pupil dilation. After complete membranectomy and visco pupil dilation, the missing zonule and deep lens position could be seen.  Challenging cataract surgery, expanding ring, and PC IOL implantation are demonstrated.
