Visual Outcome and Patient Satisfaction of Multifocal IOL With Low-Diopter Addition for Daily Life Activities Compared With Monofocal IOL

Purpose
To evaluate the visual outcome and patients’ satisfaction in eyes implanted with multifocal intraocular lens (IOL) with an inferior segment near add of +1.5 diopter(D) in comparison with conventional monofocal IOL.

Methods
This prospective, comparative, nonrandomized, consecutive case series were performed using  30 eyes of 17 patients who underwent cataract surgery with implantation of a low-add-power multifocal IOL (Lentis LS313-MF15) (multifocal group) and 40 eyes of 20 patients treated with conventional monofocal IOL (Zeiss CTS204) (monofocal group).

Results
The mean best corrected distant visual acuity (logMAR) in multifocal group was -0.07 at 5 m, -0.07 at 2 m, -0.07 at 1 m, -0.07 at 70 Cm, 0.49 at 50 Cm, 0.55 at 40 Cm, 0.50 at 33 Cm. The defocus curve in multifocal group had a plateau of optimum vision from 70 Cm to 5 m and near vision slightly dropped off, however monofocal group showed immediate drop around 5 m distance. Photopic contrast sensitivity for distance was reduced at high frequencies in multifocal group but comparable to that of monofocal IOLs. No patients of multifocal group reported severe difficulty with any of lifestyle activities, however, 36.2% of patients reported moderate difficulty for reading or near work.

Conclusion
The aspheric multifocal IOL with low addition power provide excellent distant visual outcome compared to the monofocal IOL. Furthermore, intermediate vision with tolerable near vision was provided and patients were comfortable in any lifestyle activities when using this multifocal IOLs.
