Video shows phacoemulsification in a patient with high axial myopia. More than 180 degrees of zonular dialysis was noted as the IOL was about to be introduced. A Cionni capsular tension ring was then used, and IOL implantation was accomplished. The case highlights anterior chamber fluctuation as a cause of intraoperative zonular dialysis.
