Video shows the Argentinian flag sign in a posterior subcapsular cataract with a perfect red reflex and intact zonules. The other eye was a straight forward cataract done one week earlier. Expectations of unpredictable capsular surface tension should be expected and very slow dealing with the capsule to create a circular, regular capsulorhexis that should always be aimed for. Supracapsular nuclear fragmentation is strongly recommended in these cases. An in the bag hydrophobic acrylic IOL was inserted.
