Inadvertent injection of a dexamethasone rod into the crystalline lens, across the visual axis, was managed by lens extraction with zonular support and primary posterior capsulorhexis with intravitreal placement of the drug. The initial indication for injection was a branch retinal vein occlusion which subsequently responded well to the treatment.
