IOL Vitreous Rescue and Posterior Iris Suture in Absence of Capsule Support

As a result of a PCT, a sulcus-placed IOL fell into the vitreous cavity 1 day after phacoemulsification in a female patient. The VR surgeon performed a pars plana vitrectomy and rescued the IOL. The anterior segment surgeon placed the IOL in the sulcus to perform posterior haptic suturing; however, the IOL fell again. This time, the VR surgeon broke 1 haptic and removed it. A 3-piece IOL was implanted and sutured inferiorly to the iris using a Siepser knot aided by the anterior segment microforceps and graspers.
