After proper phacoemulsification, the capsular bag was inflated with HPMC and a single-piece hydrophobic acrylic PCIOL was loaded in a B cartridge to be inserted into the capsular bag through the main incision. The plunger was pushed to deliver the IOL in a normal way, which had been done in more than 10,000 cases without complications, but in this case the IOL was dropped in the vitreous cavity by making a small inferior zonular dialysis and the posterior capsule was perfectly intact, which was unique.
