The risks of posterior polar cataract can be eliminated by use of a balanced anterior/posterior segment fluid flow, posterior by a 23-gauge pars plana infusion inserted prior to surgery with a fluid restrictor and connected to a Y tube leading in the hand piece. Fluid simultaneously flows to both the posterior and the anterior segments; thus, there is no risk for a posterior blow-out.
