Anterior capsule contraction syndrome is a complication of the capsulorhexis after cataract surgery. Extreme phimosis can affect the IOL's position and function, result in zonular dehiscence, and lead to capsule--IOL complex dislocation. Surgical capsulotomy technique will be presented as an option for an extremely thick fibrotic membrane.
